HOW TO ORDER YOUR REPORT

Please follow the instructions below to order your report and submit your photo for a your Progress Report. Make sure you read the terms & conditions before submitting and paying for your report.

STEP 1: FILL OUT THE FORM BELOW

Please complete your details in the form below.

STEP 2: PHOTO INSTRUCTIONS

Please attach a photo headshot of yourself. If your image is not appropriate to use, Lance from my team will be in touch with you.

Basically your face needs to be as straight as possible (otherwise you will get a very strange Face Split!). If you can get someone else to take the photo and follow the below instructions - it may be easier.

Make sure your chin is tucked in & your eyes are looking straight ahead
Make sure your head is straight (no tilting to the left or right)
Make sure your face is evenly lit... best to have light come towards your face rather than the side
If possible pull your hair back away from your face
Make up is ok, but keep it minimal
Keep a natural and relaxed state (a little bit like a passport photo)

STEP 3: CONFIRMATION & TURN AROUND TIME

Once your payment and photo has been received, you will be sent a confirmation email within 48 hours that will provide the turn around time for your report (usually within 7 working days).

All correspondence and your report will be sent to the email address you provide in your submission form.
